Registered Nurse in Family Care

Become a key player in VHA Home Health Care's Child and Family Program in York Region. Your nursing skills will provide essential home care to children and families in need.

As an RN or RPN, you will focus on delivering personalized, one-on-one home-based nursing services to clients aged up to 25 years.

Your role will involve ongoing assessment, care planning, and the evaluation of client health. Supported by a regional team, you'll also enjoy opportunities for professional growth and educational advancement.

Key Responsibilities:

• Assess and evaluate client health needs continuously

• Implement and coordinate care planning effectively

• Maintain thorough documentation aligned with nursing standards

• Inform supervisors of any significant client changes

• Foster a culture of continuous quality improvement

Requirements:

• Registered and in good standing with the CNO

• RN or RPN diploma from a recognized institution

• First Aid and CPR Certification required

• Experience in acute care nursing preferred

• Valid Ontario driver's license and reliable transportation
