The Medical Behavioral Integration Nurse Case Manager (MBI NCM) serves as a clinical liaison between Optum Care and its customers, integrating a collaborative process to assess, plan, implement, coordinate, monitor, and evaluate options and services to meet members' health needs.
Highlights- Pay: $60,200 - $107,400/year
- Location: Layton, UT
- Schedule: Full Time, Day shift
- Work Style: Field-based
- Screen and identify members with high-risk, long-term chronic conditions for care management services.
- Perform member evaluations and onsite visits in various settings.
- Conduct comprehensive assessments for members with high-risk symptoms/diagnoses.
- Document findings and develop individualized care plans compliant with CMS regulations.
- Utilize advanced clinical skills to meet members' health, behavioral health, and psychosocial needs.
- Document patient/family status, diagnosis, treatment plan, and progress in electronic medical records.
- Advocate for members and families to ensure their needs are represented by the healthcare team.
- Attend interdisciplinary team meetings regarding patient care and collaborate with providers.
- Facilitate MD consultation on complex cases using professional knowledge and critical thinking.
- Manage assigned case load efficiently and effectively.
- Education: Active and unrestricted RN license
- Experience:
- 4+ years of experience in nursing
- 2+ years of experience in a related field-based case management preferred
- Licenses &
Certifications:- RN (Registered Nurse) - State Licensure required
- Driver's License and access to reliable transportation
