Position Details:

Join a growing team of geriatrics providers at Hartford Healthcare and make a meaningful impact in the lives of geriatric patients! We are seeking a full-time Nurse Practitioner or Physician Assistant for our home-based REACH (Resource for Elderly and Care at Home) Program, delivering compassionate care in the comfort of patients’ homes and assisted living facilities. Role: Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA)

Location: Homecare based in the greater Manchester, CT area

Setting: Geriatric Medicine in the home setting, working within a multi-disciplinary care team

Schedule: Full-time, weekdays only!

Dedicated support and voice for our for Nurse Practitioners and Physician Assistants through the Office of Advanced Practice directed by Sarah Clark, MBA, MHS, PA-C, BSN, RN, System Director of Advanced Practice

Nurse Practitioners and Physician Assistants Role Scope:

Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physicians.

Qualifications

Requirements:

Licensure: Unrestricted State of Connecticut Medical License (or eligible)

Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA

Experience: At least two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA) in Geriatric Medicine or a related field

This is a salaried position. While compensation is presented as an hourly rate for consistency across our postings, this position is paid on a salary basis. The hiring range reflects a good faith estimate of pay for this position as of the posting date. Actual pay will be determined based on qualifications, location, internal equity, and other factors that may include hire bonuses, incentive pay plans, loan repayment, administrative stipends, or relocation.
