Hartford Health Care at Home seeks a Wound Care LPN to participate in daily wound management for our home-based program. You will perform wound care, follow evidence-based plans, and coordinate with the Wound Team RN and CWOCN to ensure quality patient outcomes.
Qualified candidates have a CT-approved LPN license and at least two years in nursing, preferably in home care. Certification in wound/ostomy is a plus, with opportunities for professional development and growth within a supportive team.
