The Columbus Organization is hiring a Clinical Intake Specialist-LPN to act as the primary contact for individuals with complex care needs. You will conduct initial visits, perform needs assessments, and coordinate care with the Case Manager to ensure continuity of care.
Role requires travel throughout Dougherty County and surrounding areas with remote administrative work when not on client visits. LPN license and 3+ years nursing experience are required;
Medicaid/Medicare knowledge is a plus.
