Care Manager; RN

The Care Manager (RN) develops, assesses, and facilitates complex care management activities for members with physical needs, focusing on high-quality, cost-effective healthcare outcomes. This role is ideal for candidates with strong physical and behavioral health experience.

• Pay: $56,200 - $101,000/year
• Location: Remote, NH
• Schedule: Full Time, Day shift

• Evaluate the needs of the member and recommend a plan for the best outcome.
• Develop ongoing care plans and collaborate with providers to address unmet needs.
• Identify problems/barriers to care and provide appropriate interventions.
• Coordinate between the member and care provider team for person-centered care.
• Provide ongoing follow-up and monitoring of member status and care plan goals.
• Provide resource support for local services based on assessments.
• Facilitate care management and ensure timely access to needed services.
• Perform outreach to assess member needs and collaborate with resources.
• Collect, document, and maintain member information for compliance.
• Provide education to members and their families on healthcare options and benefits.

Education: Degree from an Accredited School of Nursing or Bachelor's degree in Nursing

Experience: 2 – 4 years of related experience
