We’re hiring a Registered Nurse (RN) to join an established PACE (Program of All-Inclusive Care for the Elderly) team in Springfield, MA. This is a full-time, permanent opportunity supporting older adults with the goal of helping them remain healthy, independent, and safely at home

This is an excellent opportunity for an RN with acute care, home health, geriatric, or community-based nursing experience who wants a more relationship-driven approach to patient care.
Why Nurses Are InterestedMonday–Friday daytime schedule with clinic hours ~8:00 AM–4:00 PM (Occasional Weekends)
Primarily home-based skilled nursing with additional care coordination, clinic visits, and interdisciplinary collaboration

What You’ll DoAs an RN, you’ll provide skilled nursing care to PACE participants primarily in their homes while serving as an important part of their interdisciplinary care team.
Responsibilities include:
Developing and implementing individualized care plans
Monitoring health status and identifying changes in condition
Providing medications, treatments, and skilled nursing interventions
Educating patients and caregivers on medications, disease management, and care plans
Coordinating care with physicians, therapists, and other team members
Participating in interdisciplinary care planning
Maintaining accurate and timely clinical documentation
What We’re Looking ForCurrent Massachusetts RN license
Graduate of an accredited nursing program;
Associate or Bachelor’s degree required
At least 1 year of nursing experience (geriatric care, home care, or long-term care preferred)
Valid driver’s license, reliable transportation, and acceptable driving record
Comfortable providing care in patients’ homes and working collaboratively across disciplines
