Hartford Health Care Medical Group is seeking a full-time Family Nurse Practitioner (APRN) or Physician Assistant (PA) to join our hybrid remote primary care team. You will provide Transition of Care, Annual Wellness Visits, or same-day primary care visits remotely, with on-site support when required.
In this role you will also see patients virtually at two Campus Care sites and have scheduled on-site time to support the clinical team.
