Constellation Health Services seeks an experienced Hospice Nurse Practitioner to deliver comprehensive hospice care in coordination with a collaborating physician. You will conduct exams, develop care plans, and manage terminal illness in patients within home settings.
Responsibilities include face-to-face visits for eligibility, documentation, and collaborative care planning with the hospice interdisciplinary team. Prior hospice/home care experience and CHPN certification are preferred.
