This position requires the FNP to work onsite in a variety of settings, primarily focusing on home care. The ability to adapt to various environments is essential, as services are delivered in familiar surroundings for patients, thus improving their receptiveness to care. The role may occasionally extend to community events or mobile clinics, ensuring that comprehensive healthcare is accessible to all.

Key Responsibilities
  • Conduct thorough medical assessments through history taking and physical examinations, especially focusing on risk stratification and regulatory compliance.
  • Carry out diagnostic tests and procedures as clinically indicated and develop evidence-based treatment plans tailored to the individualized needs of each patient.
  • Document clinical findings accurately and communicate effectively within the healthcare team to ensure continuity of care.
  • Manage both chronic and acute primary care issues, discerning medical urgency to prioritize treatments and referrals accordingly.
  • Establish and record appropriate medical diagnoses, seeking specialist consultations when advanced care is warranted.
  • Collaborate with a supervising physician to ensure all care provided meets the highest standards and falls within regulatory requirements.
  • Engage actively in patient education, empowering individuals and families to manage their health effectively, while offering resources and referrals as necessary.
Patient Population / Client Focus

This role focuses on a diverse spectrum of patients, including adults, geriatric populations, and specialized programs such as women’s health and pediatric care. The FNP will tailor their approach to meet the unique healthcare needs of these groups, addressing their individual medical histories, lifestyle factors, and preferences to deliver optimal health outcomes.

Qualifications
  • A Master’s degree in Family Health from an accredited nursing program.
  • An active and unrestricted national certification from a recognized body such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
  • Strong communication skills and a commitment to providing compassionate care.
Preferred Experience
  • A minimum of 1 year of relevant clinical experience, particularly in chronic disease management and preventive healthcare.
  • Experience in providing care to diverse populations, including geriatrics and women, highlighting fluency in the specific health needs of these groups.
  • Familiarity with electronic medical records (EMR) systems to manage charting and patient data efficiently.
