South Shore Health System is seeking an MIH/RPM nurse to support referral screening, patient triage, and coordination across the MIH, EMS, and care management teams. The role involves patient outreach in person and by phone, triage for appropriate MIH placements, and collaboration with SNF at Home programs.
The nurse will use Epic Care Link, MyChart, Tiger Connect and other platforms, reporting to the Clinical Innovation Physician and Directors of EMS/MIH. LPN or RN licensure is required.
