Our client is looking for a highly experienced and dedicated Senior Registered Nurse specializing in Case Management to join their expanding healthcare team. • Assess patient needs and develop comprehensive, individualized care plans in collaboration with healthcare teams.

• Liaise with insurance providers to obtain authorizations for services and manage coverage issues.
• Monitor patient progress, evaluate the effectiveness of care plans, and make necessary adjustments.
• Educate patients and families about treatment options, available resources, and discharge planning.

• Current and unrestricted Registered Nurse license.
• Minimum of 5 years of experience in clinical nursing, with at least 2 years in case management or care coordination.
• Strong understanding of healthcare regulations, insurance processes, and community resources.
• Excellent assessment, planning, and problem-solving skills.
• Proficient in using electronic health record (EHR) systems and telehealth platforms.
