Care Plus Hospice Care LLC is seeking a compassionate and dedicated After Hours Hospice Nurse (On-Call) to join our team in the Phoenix AZ valley. This is a meaningful opportunity to provide expert clinical care and emotional support to patients and families during their most critical moments — after hours when they need it most. Responsibilities:- Respond to after-hours patient and family calls, providing triage, guidance, and in‑person visits as needed
- Conduct skilled nursing assessments and interventions for hospice patients in home or facility settings
- Manage and adjust pain and symptom control measures in accordance with the patient’s care plan
- Communicate effectively with the on‑call physician and interdisciplinary team regarding patient status changes
- Provide emotional support and education to patients and families during end‑of‑life transitions
- Document all clinical encounters, interventions, and patient updates accurately and in a timely manner
- Coordinate after‑hours care needs including medication changes, equipment, and emergency support
- Active RN or LPN license in the state of Arizona (RN preferred)
- Minimum 1‑2 years of hospice, palliative care, or home health nursing experience
- Strong clinical assessment and critical thinking skills in autonomous settings
- Ability to work on‑call shifts including evenings, weekends, and holidays
- Valid driver’s license and reliable transportation for patient home visits
- Proficiency in electronic medical records (EMR) documentation
- Compassionate, calm, and professional demeanor when working with patients and grieving families
