Home Health RN Case Manager: Lead Care

Elara Caring is seeking a Home Health RN Case Manager to deliver high-quality in-home care across Attleboro, Foxboro, Walpole, Sharon and Easton. You will perform skilled nursing visits, develop and update care plans, manage an assigned caseload and coordinate with physicians and clinicians to ensure patient recovery and independence.

This full-time role offers a guaranteed salary of $90,000-$115,000 plus a productivity bonus, mileage reimbursement, flexible scheduling, paid orientation,
