Centers Health Care seeks compassionate RNs or LPNs to join our Remote Patient Monitoring (RPM) team in a hybrid role. You will monitor vital signs, identify trends, and collaborate with providers to improve patient outcomes, with in-person visits twice a month.
You will work from home with a secure RPM platform, maintain accurate documentation in the EMR, and support patients managing chronic conditions while ensuring Medicare RPM requirements are met.
