The full-time Nurse Case Manager role supports our Patient Support services and requires a solid background in nursing. You will act as a vital bridge between patients and providers, guiding them through therapy options and addressing all related inquiries. Your contributions will help improve patient adherence and satisfaction.

Key Responsibilities:

• Provide disease-related information and assess patient needs

• Act as a liaison between patients and healthcare teams

• Document patient cases thoroughly in electronic systems

• Follow up on therapy adherence and disruptions

• Coordinate with specialists to seek funding solutions

Requirements:

• Diploma in healthcare (Bachelor preferred)

• Licensed Nurse (RN or RPN/LPN) with active registration

• 2-5 years of relevant case management experience

• Fluency in English and/or French may be necessary

• Strong organizational and communication skills required
