Hospice RN Case Manager - Lead Home Care
Preceptor Home Health & Hospice in Wisconsin is seeking a skilled RN Case Manager to coordinate patient care, perform assessments, and educate families. You’ll work with an interdisciplinary team to develop plans of care and ensure high-quality home-based services.
This full-time role offers a $5,000 sign-on bonus and hourly wages up to $45, with the chance to lead case conferences, supervise LPNs and aides, and arrange equipment and services for patients.
