Our Program of All-Inclusive Care for the Elderly (PACE) center in the Greater Denver Region is looking for a compassionate, skilled Advanced Practice Provider (NP or PA) to join our collaborative Interdisciplinary Team (IDT).

We are actively hiring for an immediate onboarding window to ensure a comprehensive, intentional overlap period with departing team members-giving you a smooth handoff, full panel continuity, and a stress-free orientation.

• Quality Over Quantity: Average 5 face-to-face encounters per day. Your remaining time is dedicated to care planning, family conferences, and team collaboration.

• Low Volume, High Impact: Manage a mature panel of 75-80 seniors with a gradual ramp-up.
• Work-Life Balance: Predictable Monday-Friday schedule with a dedicated weekly remote administrative day.
  • Mon-Wed (On-Site): Clinic visits, acute assessments, daily IDT meetings.
  • Thu (Community): Local nursing facility / SNF / home visits.
  • Fri (Remote): Work-from-home charting, care coordination, follow-ups.
• Light On-Call: Dedicated Nurse Triage handles first-line calls; provider escalation is rare and receives supplemental call pay.
• Autonomy & Support: Direct reporting to the Medical Director with zero RVU quotas.

• Active (or eligible for expedited) Colorado NP or PA license & DEA certification.
• AGPCNP, FNP, ACNP, or NCCPA certification.
• 2+ years experience in geriatrics, internal medicine, family practice, SNF, or home care preferred (standout new grads with strong RN/rotation background considered).

• Competitive Base Salary (based on experience) + Call Stipends.
