Focus Home Health | Redwood City, CA & Surrounding Peninsula Communities
Focus Home Health is growing our clinical team! We're seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in the comfort of their homes. This opportunity servces patients throughout the Peninsula, with a coverage area extending from North Palo Alto to San Mateo, including Redwood City, Menlo Park, San Carlos, and surrounding communities.
At Focus Home Health Families Serving Families is more than a slogan—it’s the foundation of how we care for our patients and support one another.
If you enjoy building meaningful relationships with patients, working independently in the field, and making a difference in someone’s recovery, independence, and quality of life, we’d love to connect with you!
What You'll Do- Provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN).
- Follow the established plan of care and perform ordered preventative, therapeutic, rehabilitative, comfort, and pain-management interventions within the LVN scope of practice.
- Monitor patients for changes in condition and promptly communicate significant findings to the RN, physician, and other appropriate members of the care team.
- Perform treatments and procedures such as medication administration, injections, wound care, vital-sign monitoring, and other skilled nursing services as ordered.
- Educate patients and caregivers regarding medications, treatments, disease management, safety, and the plan of care.
- Assess the home environment for factors that may affect patient safety and make appropriate recommendations to the care team.
- Collaborate with patients, families, caregivers, and interdisciplinary team members to coordinate services and promote positive patient outcomes.
- Obtain and document verbal orders from healthcare providers as appropriate and within the LVN scope of practice.
- Maintain timely, accurate, and complete clinical documentation.
- Help patients achieve greater independence, remain safely at home, and reduce the risk of avoidable hospitalization.
We’re looking for an LVN who combines strong clinical skills with compassion and a genuine commitment to patient-centered care. You are comfortable working independently in a home-based environment while staying closely connected to your clinical team. You understand that home health is about more than completing a visit—it’s about getting to know your patients, recognizing changes in their condition, educating families and caregivers, and helping each patient achieve the best possible outcome.
