the Home Health LVN for the Denton/Aubrey/Prosper area will provide direct nursing care to patients in their homes, including skilled assessments, medication administration, wound care, and monitoring of chronic and post-acute conditions. This full-time role is based in the North, with field visits in the designated service areas and some work-from-home responsibilities for documentation, care coordination, and follow-up communications. The LVN will collaborate with registered nurses, therapists, physicians, and other care team members to implement individualized care plans and respond to changes in patient status.

Daily responsibilities include educating patients and caregivers on disease management and home care procedures, maintaining accurate clinical records in accordance with regulatory standards, and supporting safe transitions of care. The role also involves participating in interdisciplinary case conferences and contributing to a culture of patient-first, evidence-based practice.

• Active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state of Texas, in good standing.
• Demonstrated Nursing skills, including patient assessment, clinical documentation, and care plan implementation.
• Experience or training relevant to home health, such as chronic disease management, post-surgical care, and symptom monitoring.
• Background in Home Care, including in-home visits, safety assessments, and coordination of community-based services.
• Strong Patient Care capabilities, with a focus on compassion, dignity, and culturally sensitive communication.
• Previous home health, hospice, or palliative care experience preferred.
• Ability to travel within the Denton/Aubrey/Prosper area; valid driver’s license and reliable transportation required.
• Proficiency with electronic health records and basic computer skills.
• Effective time management, organization, and ability to work both independently and within an interdisciplinary team.

• Pay starting at $42/visit, or $63k/annually, and increases with relevant experience
