Cadence is hiring an Registered Nurse to own after-hours patient monitoring, triage, and clinical escalation for our chronic care population. This independent role operates when primary care offices are closed, covering hypertension, heart failure, and diabetes care needs.
You will work directly with patients and the Cadence care team to ensure continuity and safety across every after-hours interaction while documenting in the Cadence platform and Zen Desk.
