Hospital Safe Discharge RN Case Manager - Remote

The Hospital Safe Discharge RN Case Manager initiates proactive outreach to facility staff to manage discharges, working collaboratively to resolve discharge barriers and ensure a smooth transition for patients.

• Assess barriers to a safe and efficient discharge process.
• Coordinate and drive an efficient discharge process with stakeholders.
• Demonstrate knowledge of acute inpatient case management and care coordination.
• Manage assigned case load efficiently utilizing time management skills.
• Prepare and lead periodic rounds to discuss case needs and prioritize discharges.
• Utilize electronic platforms and EMR systems for patient care coordination.
• Confer with Medical Director or leadership for complex case reviews.
• Identify and communicate a member's level of risk and participate in planning.
• Collaborate with hospital staff, patients, families, and community services.
• Support hospital staff in facilitating member transitions to post-acute care.

• Bachelor of Science in Nursing required

• 5+ years of nursing experience in an acute care setting required
• 2+ years of discharge planning experience required

• Current, unrestricted RN license - Compact State License required
• Case Management certification preferred
