At Cencora, this full-time position plans and manages complex cases within patient services. You will assess needs, support care planning, facilitate transitions, and coordinate with providers, pharmacies, payers, and internal partners. The work also includes identifying recurring issues and improving documentation consistency.

• Plan and manage complex care coordination cases
• Conduct assessments and support individualized care planning
• Facilitate transitions addressing clinical and access barriers
• Coordinate providers, pharmacies, payers, and internal partners
• Identify root causes and recommend process improvements

• Bachelor’s degree in nursing or related field
• Equivalent experience may satisfy educational requirements
• Four or more years in care management
• Experience in case management or patient services
• Relevant certification preferred
