Registered Nurse Patient Support Manager

Deliver patient-centered support for Shoppers Drug Mart in Southwestern Ontario through case coordination, telephony, education, and advocacy.

This Nurse Field Case Manager serves as a primary contact for patients, physicians, healthcare professionals, and vendors. The full-time position manages a part-time caseload, coordinates services, supports reimbursement, and maintains detailed patient documentation throughout therapy. Work is home-based, with travel primarily around London and surrounding communities.

• Act as the primary resource for patients and care teams
• Facilitate services between clinics, pharmacies, and departments
• Provide face-to-face and virtual patient education
• Report adverse events and product technical complaints
• Advocate for patients and families while respecting confidentiality

• Registered Nurse license unrestricted in employment province
• Strong assessment, communication, critical thinking, and decision-making skills
• Proficient understanding of reimbursement landscape
• Strong telephony and computer application skills
• Organized approach to case management

Use clinical judgment, patient advocacy, and coordinated communication to support therapy outcomes across the assigned Ontario territory.
