Karing Hearts Home Health is looking for a compassionate and skilled Per Diem RN to join our dedicated home health team in Lake Forest, CA. This is a fantastic opportunity for a registered nurse who thrives on delivering personalized, one-on-one patient care in the comfort of patients' homes. Responsibilities:- Conduct skilled nursing assessments and develop individualized patient care plans
- Administer medications, wound care, IV therapy, and other clinical treatments in the home setting
- Monitor and evaluate patient health status, reporting changes to physicians and care team members
- Educate patients and family members on disease management, medications, and care techniques
- Coordinate with interdisciplinary team including therapists, aides, and case managers
- Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards
- Ensure patient safety and comfort while promoting independence and quality of life
- Active California Registered Nurse (RN) license in good standing
- Minimum 1 year of clinical nursing experience; home health experience preferred
- Knowledge of Medicare home health guidelines and OASIS documentation a plus
- Current CPR/BLS certification required
- Valid California driver's license and reliable transportation
- Strong communication, time management, and critical thinking skills
- Compassionate, self-motivated, and comfortable working independently in the field
