Karing Hearts Home Health is looking for a compassionate and skilled Per Diem Registered Nurse to join our dedicated home health team in Ventura, CA. Responsibilities:- Conduct skilled nursing assessments and develop individualized patient care plans
- Administer medications, wound care, IV therapy, and other clinical treatments in the home setting
- Monitor and evaluate patient health status, reporting changes to the physician as needed
- Educate patients and family members on disease management, medications, and home safety
- Coordinate care with interdisciplinary team members including therapists, aides, and physicians
- Accurately document patient visits, progress notes, and care plans in compliance with Medicare/Medicaid guidelines
- Ensure compliance with all state, federal, and agency home health regulations
- Active California Registered Nurse (RN) license in good standing
- Minimum 1 year of clinical nursing experience; home health experience preferred
- Current BLS/CPR certification
- Valid California driver's license and reliable transportation
- Familiarity with OASIS documentation and Medicare home health guidelines is a plus
- Strong communication, time management, and critical thinking skills
- Compassionate, self-motivated, and comfortable working independently in the field
