Care Manager; RN

The Care Manager (RN) at Centene develops and facilitates complex care management activities for members with physical needs, ensuring high-quality, cost-effective healthcare outcomes. This remote position is ideal for candidates with experience in OB, NICU, and postpartum care.

• Pay: $56,200 - $101,000/year
• Location: Remote, WI
• Schedule: Full Time, Day shift

• Evaluate the needs of the member and recommend care plans for the best outcomes.
• Develop ongoing care plans and collaborate with providers to address unmet needs.
• Identify problems/barriers to care and provide appropriate interventions.
• Coordinate between the member and care provider team for person-centered care.
• Provide ongoing follow-up and monitoring of member status and care plan goals.
• Offer resource support for local services based on assessments.
• Facilitate care management and ensure timely access to needed services.
• Perform telephonic, digital, and home outreach to assess member needs.
• Collect, document, and maintain member information for compliance.
• Educate members and their families on disease processes and healthcare options.

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required.

• 2 - 4 years of related experience required.

• RN

  - State Licensure and/or Compact State Licensure required.
