Nurse Care Coordinator; Fully Remote

• The Nurse Care Coordinator (RN) is an integral member of Clarity Pediatrics' interdisciplinary care team.
• This role ensures seamless care delivery for children with chronic conditions and serves as a primary point of contact for families, providers, and pharmacies.

Key Responsibilities:

• Coordinate clinical appointments, referrals, and follow-ups to reduce gaps in care.
• Provide patient and family education, manage medication authorizations, and handle pharmacy coordination.
• Document clinical interactions and participate in interdisciplinary case conferences.

Qualifications:

• Active California Registered Nurse (RN) license in good standing.
• Strong organizational, communication, and interpersonal skills with tech-savvy proficiency in EHR systems.
• Bilingual English/Spanish and comfort working with diverse pediatric populations.

Logistics:

• Full-time remote position, Monday-Friday 9AM-6PM PST.
• Salary range $75,000 - $90,000/year with medical and ancillary benefits.
