About Chorus Health

Chorus Health is an embedded operating partner that helps health systems and community oncology programs launch and run advanced therapy programs including bispecific antibodies, CAR-T, and cell and gene therapies within their existing infrastructure. Our model combines a specialized clinical workforce, an AI-enabled operating system, and working capital that bridges program launch to reimbursement. This role is central to our clinical workforce pillar: delivering structured remote patient monitoring that allows partner sites to run advanced therapy programs safely in the outpatient setting.

Basic Function and Scope of Responsibilities

Responsible for delivering remote patient monitoring for patients receiving bispecific antibody and CAR-T therapies across Chorus Health's partner network. Working from our New York office in collaboration with the lead APP and Chief Medical Advisor, the RN assesses patients by telephone and video at protocol-defined intervals, identifies early signs of treatment-related toxicity through the live data-stream from remote patient monitoring devices, and escalates to the clinic on-site team appropriately to protect patient safety.

Principal Responsibilities
  • Coordinate treatment starts with partner-site physicians, physician teams, and patients to align remote monitoring schedules with bispecific step-up dosing and CAR-T administration timelines, whether care is delivered in the outpatient or inpatient setting.
  • Provide remote (telephone and video) monitoring to patients at protocol-defined intervals, documenting each encounter accurately and promptly in the appropriate system of record.
  • Assess patients remotely using record review, verbal and visual cues, patient-reported vitals and device-captured vitals, ICE questionnaire, patient condition, and medical history.
  • Determine the patient's needs and the urgency of those needs at each monitoring call based on the remote assessment, record review, vitals, and ICE questionnaire, and provide guidance within the RN scope of practice.
  • Monitor for early signs and symptoms of cytokine release syndrome (CRS) and immune effector cell–associated neurotoxicity syndrome (ICANS), grading severity and escalating per protocol.
  • When a patient needs to be evaluated in person, coordinate promptly with the partner site's clinical team and care pathway to ensure timely assessment and treatment at the designated location.
  • Ensure accurate notes of all calls and confirm they are documented in the correct system of record and communicated to the appropriate care team.
  • Provide patient education regarding the oncology disease process, the side effects of bispecific and CAR-T treatments, warning signs to report, and healthful behaviors as needed.
  • Serve as a clinical resource and professional role model for other members of the monitoring team.
  • Communicate appropriately and tactfully with staff, consultants, patients, family members, and partner-site collaborators.
  • Provide guidance to partner-site physician teams and staff onboarding new patients to the monitoring program as needed.
  • Participate in staff in-service, case review, and other team meetings as needed.
  • Always prioritize patient safety.

Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions of this position.
