Provide one-on-one home health care in Sleepy Hollow as a Registered Nurse or Licensed Practical Nurse. Work part-time, supporting one patient per shift.
Elite Care Management seeks a Skilled Nurse for home-based patient care. The assignment focuses on daily living activities, comfort, and overall well-being. Registered Nurses and Licensed Practical Nurses plan, organize, and assist with services according to each patient’s care plan.
Key Responsibilities:- Observe patients and record significant conditions or reactions
- Notify physicians or supervisors about patient changes
- Manage care according to the patient care plan
- Administer prescribed medications and treatments
- Monitor and document intervention effectiveness
- Current Registered Nurse or Licensed Practical Nurse state license
- Current CPR card
- Driver’s license and auto insurance card
- Social Security card
- Required TB testing documentation
Use nursing communication, treatment administration, and care planning skills while supporting one Sleepy Hollow patient throughout each shift.
