Remote - Visiting RN — On-Call & Home Visits

Innov Age Holding Corp. is seeking an On-Call Visiting After Hours Registered Nurse (RN) to respond to after-hours calls, triage care, and perform home visits when necessary. This remote, salaried role collaborates with the In-Home Services and Primary Care teams within the PACE program.

Requirements include an active RN license, First Aid/BLS, and 2+ years in geriatrics with home health experience. Mileage reimbursement applies for travel.
