Case Manager; Fully Remote

• Collaborate across teams to deliver high-quality care and improve member health outcomes.
• Educate members, evaluate Social Determinants of Health, and connect individuals with community resources.

• Coordinate care across settings and maintain strict confidentiality and high professional standards.
• Provide crisis intervention support and act as a dedicated member advocate.
• Engage with members via phone, secure messaging, or in-person to provide education.

• 2+ years of clinical experience in hospital, outpatient, or community based care management.
• Active, unrestricted RN licensure from the United States or a Master's in Social Work from an accredited school.
• 1+ year experience in care coordination and navigation.
