Sentara Health Plans is seeking a Care Coordinator- LPN to provide case management for Members in Nursing Facilities across Roanoke, Lynchburg and Danville areas. The role is remote from home with periodic travel to facilities; typical hours are Mon-Fri 8:00am–5:00pm.
Ideal candidates have an Associate's degree in Nursing (preferred) and LPN certification;
Medicaid/Medicare/HCBS experience and knowledge of HEDIS are a plus.
