Health Snap is hiring New York licensed LPNs to support patients enrolled in chronic care management and remote patient monitoring programs. You will work with the patient’s care team to educate, monitor, and empower patients to manage their chronic conditions.
The role operates Monday through Friday, 9:00am to 5:30pm Eastern Time in a full-time capacity. You’ll engage with patients, coordinate care, and ensure HIPAA-compliant, high-quality support across care teams.
