Paradigm is seeking a compassionate and organized LPN Post-Acute Care Coordinator to support injured workers through the coordination of post-acute care services. In this fully remote role, you'll collaborate with healthcare providers, case managers, and insurance professionals to facilitate care transitions, coordinate services, and ensure positive outcomes for injured workers.
This is a fully remote, full-time position working Monday through Friday from 12:00 PM to 8:30 PM Eastern Time.
