Humana is hiring an RN Care Manager to support Wisconsin Family Care Program members in Sauk County. The role is remote from home with travel up to 40% to visit members, Monday–Friday, 8:00 am–4:30 pm CST. You will assess, coordinate and educate to help members navigate their care.
Minimum qualifications include an Associate degree in nursing and Wisconsin RN license; 1 year care management experience is preferred. Mileage reimbursement is provided and TB screening applies.
