Remote Triage Nurse: Care Coordination

Vail Health Hospital is seeking a Triage Registered Nurse to perform care coordination and triage nursing activities. The role includes assessing patient health concerns, assigning dispositions to emergency, urgent care or clinic visits, and supporting providers with triage via calls and messages.

You will document assessments in the electronic health record, participate in care coordination teams, and contribute to quality improvement initiatives.
