Humana is seeking a Field Care Manager Nurse 2 to assess and guide members toward appropriate care resources, focusing on physical, environmental, and psycho-social health. You will report to the Manager, Care Management and support partners across remote settings.
Required:
BSN with SC license, 2+ years in care management, 45 hours Behavioral Health training, and EMR proficiency. Travel up to 50% and a Monday–Friday schedule with travel to offices for training as needed.
