Remote LPN​/LVN - Patient Enrollment Specialist

Chronic Care Staffing (CCS) is hiring Remote Enrollment Coordinators to help patients discover healthcare programs designed to give them more support between office visits.

You’ll talk with eligible patients, build rapport, explain the value of Chronic Care Management (CCM) and Remote Patient Monitoring (RPM), answer questions, and help them get enrolled.

Active LPN or LVN license (compact)

Current BLS

Recent healthcare or patient-facing experience

Strong communication and computer skills

Private, HIPAA-compliant home workspace

Healthcare outreach, enrollment, sales, CCM/RPM, or customer service experience is a plus - but we’ll train the right person.

Estimated Annual Compensation: $38,480–$42,640 plus bonus opportunities
