Base pay range$40,000.00/yr - $41,500.00/yr

If you would like to be a part of our team and make real difference in people’s lives. The Child and Family Guidance Center is currently seeking a full time Spanish Speaking Community Support Worker.
(Case Manager)
The Community Support Worker is responsible for partnering with families to identify strengths and needs of the child/family and advocate for appropriate support and services using the wraparound values and process. They will work with other providers to develop a Plan of Care that reflects the family’s values and culture. This position is located within our Bridgeport office but may require working out of our Norwalk location as needed and traveling within lower Fairfield and New Haven County.
The selected individual will be responsible for but not limited to the following Essential Functions:
- Provide services to about 11-13 families to identify strengths and needs.
- Assists the family in developing and convening Child and Family Team Meetings (CFT) of traditional and non-traditional providers for the purpose of developing a Plan of Care (POC) that reflects the family’s values and culture.
- Works in conjunction with DCF to receive referrals and collaborates on transition plans for Community Support Services
- Gather and submit data required by DCF in a timely manner
- Maintains a Community Support Record on all families receiving Community Support Services in keeping with the wraparound process (family driven, strength-based, culturally responsive) and outcome focused documentation.
- Provides case management services to families, coordinates child and family teaming activities assuring that families are connected to community resources.
- Offers an array of services/supports to the families to enhance protective factors and collaborates with community resources and providers. That ensures a seamless and well coordinated system of care to support the family.
- Develops a safety plan for each family served that guides the family to appropriate assistance. Ensures that service and safety plans are implemented appropriately and adapted by the CFT.
- Assists in program evaluations such as baseline and outcome measures and client satisfaction surveys to families during service delivery or upon discharge from Community Support Services
- Provides linkage and monitors coordination of supports and services, including follow-up and advocacy for children/youth and families.
- Provides weekly home-based, in-person and/or phone contact with families as indicated.
- Participates in monthly Community Support for Families staff meetings
Qualifications
- This position requires a Bachelor’s Degree in Social Work, Sociology, Psychology or in the Human Service field.
- A driver's license and reliable transportation are required.
- Travel to and from family homes is required on a regular basis working within lower Fairfield and New Haven county.
- A bi-lingual Spanish speaker is required.
- Must be fully vaccinated for COVID-19 virus, unless eligible for a medical or religious exemption.
- The Individual must possess experience providing direct services to families with complex needs.
- Ability to utilize a strength-based approach and demonstrate a non-judgmental and empathic attitude and approach with families and providers.
