Job Summary

At Catholic Charities of Ingham, Eaton, and Clinton Counties, every team member plays an essential role in advancing our mission to serve vulnerable individuals and families with compassion and dignity. As part of our team, you will contribute to a wide range of services aimed at fostering hope, healing, and empowerment within the communities we serve. As a Preferred community case manager, you will provide intensive case management to refugees and other eligible populations facing complex challenges.

You will guide clients through the intake and eligibility process, develop individualized service plans and coordinate critical supports such as medical care, benefits enrollment, and community resources. This role requires strong organizational skills, cultural humility, and the ability to navigate diverse service systems.

Key Responsibilities
  • Receive and review Preferred Communities referrals; schedule initial intake meetings.
  • Conduct eligibility assessments and open intensive case management services for qualified clients.
  • Develop individualized service plans and coordinate medical, social, and community resources.
  • Assist clients with accessing public benefits, disability services, and other essential supports.
  • Arrange and track medical appointments, referrals, and follow-ups.
  • Maintain accurate, timely case documentation, notes and reports in compliance with agency and grant requirements.
  • Collaborate with internal teams, external partners, and interpreters to ensure effective service delivery.
  • Empower clients to achieve greater self-sufficiency through advocacy, education, and navigation of community systems.

Qualifications
  • Education:

    Four years Bachelor’s Degree in any fields (Required)
  • Experience:

    worked with vulnerable population (Preferred)
  • Required Licenses, Certifications, and Training:
    • Fluency in a second language (Somali, Swahili, Arabic, Kinyarwanda)
    • A valid Michigan Driver License, access to reliable transportation and maintain appropriate insurance
    • Prefer ability to work beyond the parameters of posted Agency/Program hours of operation as necessary to complete Job Responsibilities.

Work Environment & Physical Demands
  • This position is on-site and may not be eligible for remote work.
  • Attendance at occasional meetings or events outside regular business hours is required.
  • The role involves working with sensitive or potentially traumatic information and direct interaction with individuals or families in crisis.
  • Physical requirements include frequent use of hands for handling objects, tools, or controls; talking, bending, squatting, twisting, climbing, kneeling, reaching, and occasionally lifting or carrying up to 30 pounds.
  • Vision requirements include close‑up work, distance vision, and the ability to adjust focus.
  • The work environment may involve varying degrees of physical discomfort and occasional loud noise.
  • Occasional driving, potentially with clients, during the workday may be needed for operational purposes.
