Position Overview

PATH is seeking a passionate Case Manager for our Vermont Villas site in Gardena, California. As part of the SHS team, you will provide Intensive Case Management Services (ICMS) in a Permanent Supportive Housing (PSH) setting to individuals with a history of chronic health, mental health, and/or co‑occurring conditions.

Responsibilities
  • Conduct client intake and individualized needs assessment; collaborate to develop an Individualized Service Plan (ISP) that addresses housing goals, income, and personal goals including medical, mental health, substance use, financial resources, vocational, and social support needs.
  • Provide referrals, linkages, information, and support to resources that help clients achieve their ISP goals.
  • Complete program specific assessments (e.g., VI‑SPDAT).
  • Evaluate strengths and challenges for short‑term and long‑term goals; conduct a 90‑day review with the client after initial intake.
  • Maintain documentation standards as required by PATH policies, including timely case file notes and a complete, accurate, and current client file with all required documents.
  • Maintain confidentiality of client files per HIPAA and all applicable guidelines.

  • Ethics and Boundaries: maintain ethical and professional standards, demonstrate honesty and integrity, and uphold professional boundaries.
Preferred Qualifications
  • Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field.
Minimum Qualifications
  • All levels of education and experience are welcome.
Mandatory Requirements
  • Employment eligibility verification.
  • CPR/First Aid training or ability and willingness to obtain it.
  • Successfully complete Tuberculosis Test, Background Screening, and Drug Test as conditions of hire.
Driving Requirements
  • Reliable transportation, valid driver’s license, proof of insurance and ownership for personal vehicles used during work duties, and ability to qualify for PATH’s insurance coverage.
Employment Eligibility

California‑Based Work Expectations

All work must be performed within California. PATH does not offer fully remote positions; any off‑site work is limited to approved off‑site locations and does not imply work from home.
