Case Manager – Scott County
Pinnacle Services is an innovative agency experiencing rapid growth in the Case Management department. We are seeking friendly, self‑starting professionals to join our Hennepin County Case Management team. The role focuses on delivering person‑centered services to individuals with developmental disabilities, serious and persistent mental illness, substance use issues, physical disabilities, or who are aging, primarily in the seven‑county metro area.
Responsibilities- Drive to clients in Scott County, averaging four client meetings per week.
- Assess the various areas of a client’s life that require assistance.
- Offer and coordinate resources for the individuals being supported.
- Monitor service delivery and collaborate with providers to ensure clients’ needs are met.
- Attend regular in‑person interdisciplinary team meetings with clients.
- Conduct service‑plan development and revise as necessary.
- Maintain case notes and create billable units.
- Graduate from an accredited four‑year college with a major in social work, psychology, sociology, or a closely related field.
- Or graduate from any major with one year of experience as a social worker, case manager, or care coordinator in a public or private social service agency.
- Valid driver’s license with an acceptable driving record and personal vehicle covered by current insurance.
- Successful completion of local and/or state background checks.
- Willingness to commute to various areas of Scott County for client meetings.
- Licensed Social Worker (LSW) or pursuing licensure is preferred.
Non‑LSW: $49,000/year (salaried, 40 hr/week)
LSW: $51,600/year (salaried, 40 hr/week)
