Outreach staff actively connect with people experiencing homelessness to build trust, reduce barriers, and link them to essential services.

The Outreach Case Manager plays a key role in helping clients move from the streets into stable, permanent housing, often within 30 days. This is an exciting opportunity to make a direct, meaningful impact on the lives of some of Fairfax County’s most vulnerable residents.

  • Assess housing barriers faced by individuals experiencing homelessness to identify the client’s housing and support service needs.
  • Develop individualized case management plans focused on housing acquisition, budgeting, and self-sufficiency, beginning with intake interviews to assess household needs, goals, and eligibility.
  • Utilize the Housing First model to deliver housing stabilization services and divert households from entering emergency shelters.
  • Establish and maintain consistent verbal and written communication channels, among operations staff, referral sources, and collaborating agencies to ensure effective coordination and information sharing.
  • Advocate for and actively assist individuals in accessing a range of services, including mental health, intellectual disability support, substance use treatment, housing referrals, financial assistance, home-based services, training, medical care, mentoring and socialization.
