Permanency Planner - Homefinder

Department: Foster Care

Position Summary: Work collaboratively with foster parents, biological families, referring agencies, and other community resources to ensure continuity of treatment planning toward established discharge goals.

Gateway Longview’s Foster Care Department works with foster families and youth to achieve permanency. Currently serving approximately 75-85 families with a mix of foster, adoptive, and kinship homes. Services children ages birth to 21 and tailors each case through a unique lens.

Compensation: Bachelor's Degree: $21.50 - $23.59 per hour;
Master's Degree: $23.30 - $25.38 per hour. Offers are based on education and years of relevant experience.

Schedule:

Full‑time, Monday – Friday. Some evenings may be required as needed to assist families and program needs.

Position

Specific Duties / Responsibilities
  • Implement placement development and guide a treatment plan consistent with the permanency plan of record; assist in developing a concurrent plan for each youth and family.
  • Establish and maintain regular contact and therapeutic intervention with all parties (children, birth families, foster/adoptive families) ensuring diligent efforts toward the identified service plan.
  • Provide ongoing collaboration with referral sources and community providers (e.g., Family Court, Department of Social Services, Schools, Counselors).
  • Provide after‑hour support and crisis intervention when necessary.
  • Maintain accurate, timely, comprehensive case records in accordance with ASFA regulations.
  • Screen prospective foster/adoptive parent candidates, conduct orientation sessions, pre‑certification training, and complete necessary home studies promptly.
  • Coordinate ongoing in‑service training and enrichment programs.
  • Review and document re‑certification studies for resource parents.
  • Participate actively in professional and community organizations for current trends and needs in child and family treatment.
  • Strengthen skills through ongoing in‑service training to meet minimum yearly training requirements.
  • Engage in training opportunities, group consultations, etc., to implement Solution‑Focused Trauma‑Informed Care Principles (empowerment, safety, choice, trustworthiness).
  • Other duties as assigned.
Knowledge, Skills, and Abilities
  • Excellent communication skills.
  • Multi‑tasking and detail orientation.
  • Ability to work independently and as an enthusiastic team player.
  • Availability for non‑traditional hours for emergency support.
  • Assessment skills and ability to engage families and establish relationships with social services and other providers.
  • Specialized trauma treatment training preferred.
  • Effective teamwork with clients, staff, families, and community contacts.
Qualifications
  • Bachelor’s Degree with relevant experience in social work or a related human services field.
  • Valid New York State driver’s license, adequate auto insurance, and reliable transportation for local travel.
