View Point Health has partnered with The Department of Behavioral Health and Developmental Disabilities (DBHDD), to provides support to the families of youth identified through the Coordinator, assisting them through the continuum of care. This position offers support, education, resource connection, and advocacy assistance to families of youth with Autism Spectrum Disorder (ASD) diagnoses who are at risk for repeated inpatient stays, seeking to remove barriers to long-term recovery at home and in the community.
This position will also provide consultation and expertise to ensure the course of care is coordinated, scheduled, and documented so all program participants are managed appropriately.
The MATCH Autism Connection Coordination Case Manager (CM) is creative and resourceful, driven by a heart for service, and passionate about helping youth with behavioral health needs to remain in stable recovery with their families and in the community. Serving a caseload of approximately 10 families at any given time, the Coordinator will assist families identified as needing extensive assistance by engaging them in a meaningful way from the point of intake to build trust, identify needs, and collaborate in developing a strength-based, individualized plan of support for the youth.
The Coordinator guides the family through addressing the medical, behavioral, social, educational, and other needs of the youth and family to better achieve self-sufficiency. This is achieved through client advocacy, case management, resource coordination, and linkages and/or referrals to state and local therapeutic services, community resources, and other supportive services as needed. As support and services are put in place, a strong the Coordinator will help reduce a child's risk of out-of-home placement, such as repeated hospitalizations, therapeutic foster care, or Youth Detention Centers.
- First point of contact, assess needs, develop care/support plans, provide crisis intervention, offer guidance on policies, and facilitate family communication.
- Schedule appointments, manage intake/documentation, track waiver/funding processes, and coordinate with therapists, healthcare providers, and social services.
- Inform families about available resources, conduct public awareness, build community partnerships, and advocate for individuals' rights.
- Assist in creating and implementing support programs, developing educational materials, and ensuring quality of services.
- Educate families about eligibility criteria, required medical documentation, and levels of care expectations.
- Assist families in resolving issues related to denials, requests for additional information, or re-evaluations.
- Maintain detailed case records, track progress, and prepare reports for transparency and accountability.
- Work with internal staff (therapists, billing) and external agencies (social services, legal) to ensure comprehensive support.
- Stay current on state policy, insurance guideline changes, and communicate updates promptly.
- Engage with community partners and organizations to strengthen resource networks and improve family access to services.
- Document all communication and support activities accurately in internal systems as specified in VPH policy.
- Other related duties as assigned.
- High School diploma/GED and three (3) years of job-related experience in a social services-related position; or one (1) year of experience at the lower-level social Services Technician 2 (SST
011) or position equivalent.
- Bachelor's degree in a related field from an accredited college or university AND Two (2) years of professional experience in a human services field.
- Knowledge of community resources.
- Ability to work independently and as a team member.
- Good verbal and written skills.
Competencies:
- Must be able to lift 20 pounds.
- Requires long periods of sitting/standing.
- Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
- Candidates for selection must pass a criminal background check (including fingerprinting).
- Must pass a pre-employment drug screen and subject to random drug screens.
