Full‑time – 37 hours per week.
Permanent position.
Role Overview’An exciting opportunity has arisen for the above positions. You will work closely with a Multi‑Disciplinary Team to provide Statutory Social Work. You will work with individuals, carers and their families supporting discharge to assess, flow through enablement care and admission avoidance, following discharge from hospital. Using care closer to the community principles, the postholder will work with a wide range of individuals who will have had a period of hospital stay or to prevent hospital admission.
You will be a key player in maintaining strong partnerships with health and third sector colleagues to promote a community and asset‑based approach aimed at maintaining people’s independence in their own homes; minimising the impact de‑compensation which affects both physical and mental wellbeing. You will undertake statutory social work in line with the Care Act, Mental Capacity Act and Mental Health Act.
