Foster Care Case Manager at Webster Cantrell Youth Advocacy Location: Decatur, IL Status: Full-Time, Exempt Reports To: Foster Care Supervisor

What You'll Do:
️ Assess the needs of children and families and develop individualized service plans ️ Conduct weekly home visits and maintain regular contact with foster families and other supports ️ Advocate for clients in court and through multi-disciplinary team meetings ️ Facilitate placements and provide post-placement supervision ️ Coordinate with schools, healthcare providers, and community agencies ️ Maintain accurate, timely documentation and case records ️ Participate in court hearings, administrative case reviews, and agency utilization review ️ Engage in ongoing training and professional development opportunities
What We're Looking For:
Education: Bachelor's degree in social work or a related field (Master of Social Work or equivalent preferred)
Experience: Professional experience in foster care or child welfare preferred
Requirements:
- Successfully obtain necessary foster care case management licensing
- Valid Illinois driver's license and proof of required auto insurance
- Ability to pass background checks
- Strong communication, documentation, and time management skills
- Ability to build rapport with children, caregivers, and multidisciplinary teams
- Willingness to provide home-based services and participate in on-call rotation
