Position Summary
The Wraparound Care Coordinator provides home-based services to assist youth and families within the Erie County System of Care, receiving services through the Erie County Department of Social Services. Care coordination is strength-based, individualized, focused on linkages, coordination, and monitoring of services to improve and promote family stability.
Major Responsibilities- Provides home-based services using the Mii Wrap Care Coordination model
- Utilizes a strengths-based, family-driven approach
- Partners with children, parents, guardians, the Erie County Department of Social Services, and service providers to create a comprehensive Plan of Care
- Conducts assessments throughout the case duration
- Provides education, care coordination, and community linkages as appropriate
- Conducts face-to-face contacts with families
- Conducts Child and Family Team meetings each month
- Participates in required trainings
- Maintains timely and accurate documentation in two systems
- Other duties as deemed appropriate
- Solid writing and verbal communication
- Strong engagement skills with diverse populations
- Flexibility related to scheduling
- Familiarity with computer applications (Word, Excel, Outlook)
- Familiarity with Fidelity EHR and Connections is a plus
- Bi-lingual ability is a plus
- Motivational Interviewing skills are a plus
- Bachelor’s or Master’s Degree in a human services or related field plus 1 year of professional or internship experience in a human services setting; experience must involve direct care or linkage services to at-risk youth/children and families.
- Valid NYS Driver’s License and adequate auto insurance
- Ability to work effectively with clients, families, staff, and community contacts from a variety of cultural and ethnic backgrounds.
Flexible schedule based on clients' needs.
Competitive pay rate of $21.512 per hour based on a 37.5 hour work week.
