The Case Manager is responsible for:

Assessing the areas of the consumer’s life where assistance is needed.

Offering and coordinating resources, and services for the person.

Monitoring service delivery and working with providers to ensure the consumer’s needs are met.

Provide person centered service planning with person and creating Coordinated Consumer Support Plans.

Attend regular meetings with the person and their interdisciplinary teams.

Travel to different sites in different counties for meetings.

Case managers will be providing case management for people on EW, AC, RULE 185, CADI, CAC, and IDD waivers.

Other duties as assigned.

Case Manager Requirements:

· Applicants must have a valid driver’s license and acceptable driving record as determined by Bestview Care Options policy. Applicants must have a vehicle that is covered with current insurance. Applicant must have a reliable vehicle.

· Applicants must successfully clear local and/or stated background checks.

Preferred Qualifications:

One to two years’ experience in service coordination or case management is preferred, but not required.

· Applicants will be required to have internet at home.

• Applicant must have a 4 year degree in social work, human services, or a related social science degree .

Salary: $49,000.00 - $65,000.00 per year
